Apple Valley Animal Hospital intends to proceed with this process gradually and thoughtfully, ensuring we continue to deliver timely, high-quality care to both our existing and future patients. Therefore, we kindly request that all prospective new clients complete the New Client Application form provided below. By evaluating applications ahead of time, we can carefully manage our doctors' workloads and ascertain that our hospital will meet your and your pet’s needs effectively. Please note that our receptionists are unable to expedite this process or provide further information. We appreciate your patience and understanding.

New Client Application

We hope to call approved applicants roughly 3 to 4 weeks after receipt of your application. At the time of scheduling, we will require a NON-REFUNDABLE pre-payment of the office visit fee to secure your appointment time. We do not accept applications for emergency/urgent situations as our process can take several weeks. Please contact your local emergency hospital or current veterinarian in this situation
